Birth Certificate
In order to receive a copy of your birth certificate you must contact the Department of Health (DOH) either through email (send to: Charlene Peoples, vital.records@doh.wa.gov) or mail.

Send them a letter stating that you would like to obtain your birth certificate and include the following information: Full name, Date of Birth, City and County where you were born, Father's full name, and Mother's maiden name. You should also remember to provide them with the address you would like the information sent to.

A second alternative is to go to http://www.metrokc.gov/health/vital_st/ This website will provide you with easy to follow instructions about obtaining your Birth certificate.

The cost of ordering your birth certificate is $13. If they are not able to find it, you will be refunded all but $8 of your money for a search fee.
